In order for attendees to receive SFI credit they MUST attend a minimum of 3 webinars, no exceptions will be given for those who attend 1 or 2. The attendee verification form is below. Fill it out completely, mark all webinars attended, and send it in along with a check for $35 to the FISTA address on the form.
